Non-Emergency Injuries

If this is an emergency, contact 911 or go to the nearest emergency room.
If this is a non-emergency injury or you are reporting an injury, contact your Principal Investigator (P.I.) or supervisor and follow the instructions below.

Paid Student, Employee, or Without Salary Appointment

Follow the campus wide procedure outlined in the UC Davis Injury Reporting Procedures. All work-related injuries or illnesses must be immediately reported to Workers' Compensation. Also make sure to contact your P.I. or supervisor.

Non-Emergency Situation
1. Complete the UC Office of the President Employer's First Report (EFR).
2. Print a copy and bring it with you to the clinic
3. Go to Occupational Health Services (Outside regular business hours, go to Davis Urgent Care or Sutter Davis. See hours below.)
4. Call 877-682-7778 to report the work-related injury/illness and file a Workers Compensation claim.

1. Contact your Principal Investigator (P.I.) or Supervisor (Principal Investigators (PI) should complete and submit an UC Davis Incident Report within 48 hours)
2. Go to Student Health and Wellness Center's (SHCS) Acute Care Clinic
3. Bring Insurance ID Card and Insurance Information

All students are advised to carry their health insurance ID card and have their insurance information readily available.

Reporting Work-Related Fatalities and Serious Injuries or Illness

Any serious injury that requires 24 hours or more of hospitalization, disfigurement, or loss of a body part (including digits) must be reported according the guidelines found on the UC Davis Safety Services website.

  1. From 8am-5pm Monday through Friday call Environmental Health and Safety (EH&S) at (530) 752-1493
    • After hours, call UC Police/Fire Dispatch at (530) 752-1230
  2. Complete UC Office of the President Employer's First Report (EFR). New Claim Creation and Employer Investigation guidance are available.
  3. Call 877-682-7778 to report the work-related injury/illness and file a Workers Compensation claim.
